Home Health
Home Health is committed to providing “Care with Compassion.” The patient care provided by Aleca Home Health is guided by state, federal, and local regulations, encompassing both independent and interdisciplinary care, coordinated by licensed professionals.
Hospice
Aleca Hospice provides a comprehensive and compassionate range of services, as we recognize that our loved ones all have uniquely different needs. We utilize an interdisciplinary approach to ensure patients receive end of life services that are parallel to the patient’s and family’s wishes, and these can be provided wherever the patient calls home.
Outpatient Therapy
An important part of Aleca’s Spectrum of Care is our comprehensive and personalized Outpatient Therapy program. Once your patients return home from a Short-Term Nursing and Rehabilitation Center, or if they were discharged directly home following a procedure or surgery, they can still benefit from outpatient therapy. We can help you verify insurance benefits and get your patients on track to recovery and well-being!
Work Injury Treatment
Aleca has a designated workers’ compensation team. Our team will develop a customized care plan, centered around your patient’s needs, and communicate any updates or changes with you, the adjuster and case manager, and assist with discharge planning.
Senior Living Therapy
Our highly-skilled senior care community therapy team works in conjunction with senior care communities such as skilled nursing facilities and assisted living centers to design customized therapy treatment plans.
